As a patient at Somerset Surgical Associates, we want your experience in our office to be as pleasant and effortless as possible. On this page you will find important information, policies, and documents to help ensure your visit goes smoothly.

On the day of your appointment, please bring the following:

  • Your photo identification and insurance cards
  • Insurance referral, if required
  • Completed New Patient Information Form
  • Past and current labs, radiology reports AND films, which pertain to the reason for which you are being seen
  • For patients being seen for a breast issue, you must bring the actual mammogram and/or ultrasound films. Previous years' films should also be provided.
  • If you have been previously diagnosed with cancer or have had prior bypass operations for improved circulation and are seeing the doctor for a possibly related issue, you should bring old records with information of your diagnosis and treatments.

CANCELLATION POLICY

We understand that emergencies may arise and request that if you must cancel, please give 24-hours’ notice. As a courtesy, we provide an automated reminder; however, if you do not receive it, you are still responsible for remembering your appointment. Failure to provide a 24-hour notice of cancellation will result in a $75 fee billed directly to you.

FINANCIAL POLICY

We participate with many health insurance plans. When you call for your appointment, please advise us of your current health insurance carrier. If we do not participate with your carrier, we can inquire if you have any out-of-network benefits, which may be used for your visit and treatments.

For our patients without health insurance, office visits must be paid in full at the time of your visit. Postoperative visits during the postoperative period are at no charge. Planned surgery under $750 must be paid in full prior to scheduling. For procedures over $750, at least half of the procedure fee must be paid prior to surgery, and a payment agreement must be signed for the remaining balance. Other outstanding balances must be paid in full prior to any new surgery being scheduled. If other financial agreements are necessary, please contact our billing office.

Charges from our office cover only Somerset Surgical Associates services. You may receive additional billing from laboratory, radiology, anesthesiology, hospital, or ambulatory surgery facilities.
